Medication Teaching at Home

Our nurses help patients and caregivers understand the medication plan ordered by their healthcare providers.

Support may include:

  • Reviewing medication instructions
  • Reinforcing timing and dosage directions
  • Teaching important precautions
  • Helping patients organize their routine
  • Monitoring for concerns
  • Communicating relevant issues to the healthcare provider

We do not independently change medication orders. Changes should always come from the appropriate prescribing provider.
